Online Membership Application

• Become a DCo MPC member through our online application. Follow these steps:

Step 1:

• Download and accomplish the following documents
Membership Application Form
Check Off Authorization Form
Data Privacy Form

Step 2:

• Documents needed

1. Accomplished Membership Application
2. Photocopy of any one valid ID (TIN, SSS, and other government issued ID)

Step 3:

• Send the scanned copy of your documents

• Wait for DCo MPC confirmation & instruction through official email

Step 5:

• For payments and deposits through Bank over the counter or online bank transfer

• One-Time Payment of Membership Fee Php 100.00
• Initial Share Capital of Php 500.00(succeeding payments shall be Php 100.00 only)
• Initial Mortuary Fund of Php 100.00 for dependent(succeeding payment is based on actual claims)
• CLIMBS Mortuary Insurance for members of Php 220.00 annually

Step 6:

• Scan the deposit slip and send through email as proof of deposit.
