What is Coop Loan Protection Plan (CLPP)?
- Coop Loan Protection Plan (CLPP) is a group life protection plan specially designed for the
borrower-members of the cooperative. It relieves beneficiaries of the financial obligations in case of death as DCo MPC and CLPP takes care of paying for their loans. Every full-fledged regular and associate members who has current loan availed must be enrolled in CLPP.
What are the requirements in claiming mortuary assistance?
- For death of a members:
- Certified True Copy of death Certificate
- Barangay Certificate
- Proof of beneficiary
- Claimant’s statement** with 1 photocopy of valid ID with 3 signatures
- 1 photocopy of valid ID with 3 signatures of claimant’s witness
- Identification** with 1 photocopy of valid ID with 3 signatures
- 1 photocopy of valid ID and 3 signatures of witness of #6 requirements
- Attending Physicians Report (APR)*
*waived for those members whose coverage belongs to the first remittance (members on or before February 29, 2024)

- PSA copy of Birth Certificate
- PSA copy of Marriage Certificate
- Union Certificate ( for Expansion Areas only )

What is GAD?
- The 1987 Constitution, Article 11 Section 14 in the Declaration of Principles provides that ‘’The State
